This document is used to record changes to the baseline, including the change type, priority,
the owner's name, date submitted, if escalation is required, the date it was approved, and the
action / resolution of the change.
The log consists of the following topics:
Change number
Change type
Change description
Priority (critical, high, medium, low)
Owner
Date submitted
Is escalation required?
Date change approved
Action or resolution.
