Purpose:

This template provides capabilities to document a person being hired by your company:

Employee information including the employee name, position, phone and email addresses, and his/her manager's name, phone number and email address.
Necessary orientations by the Personnel department.
Employment contracts including duration of employment, salary, etc.
Employment legal requirements to be fulfilled including preparation of W-4 and W-9 forms, direct deposit, any verification systems, etc.
Company requirements including non-disclosure and non-compete agreements, drug/alcohol testing, leave of absence, confidential and security agreements, etc.
General information provided to the employee including where the employees can find the mailroom, copy center, kitchen, cafeteria, emergency exits, etc.
An explanation of benefits packages provided to the employee, including life and health insurance, retirement plan, disability insurance, paid time off, vacation time, etc.
Work related items that are provided including a building key, cell / smart phone, credit cards, ID badge, laptop, flash drives, etc.
Recording of the employee's personal information including the employee's medical history, allergies, birthday and emergency contacts.
